VERY IMPORTANT COMPLETE AND EMAIL THE FORM BELOW

Candidates are required to email the following 3 items:

1. Submit their CIPR number when registering for the webinar sessions
2. Submit a copy of a government issued photo ID (front & back sides)
3. Submit a signed copy of the Statement of Understanding of Applicant (attached below)
